This page is intended to ease the process by which you can schedule a private nutrition consultation with me. Here you'll find a Health History Questionnaire that should be downloaded, completed and returned prior to your first appointment. The Questionnaire should be submitted along with any recent blood work or test results at least 48 hours prior to our meeting. I will review your materials in detail, getting to know you at a deeper level. This enables us to make the most of your first session and allows you to walk out the door (or hang up the phone) with some clear steps for moving toward your goals.

The comprehensive review of your materials and initial one-hour counseling session is offered at a rate of $250. This phone or in-person session is our time to discuss your health, your goals and your concerns, and to determine how we might best work together to find your unique remedy.
